About the Company

This company is a well recognised health care provider with a strong focus on community health services. We work collaboratively with a range of medical practitioners - internal and external to support the wider community and their families with medical health issues.

Role Purpose

In this role, you will be delivering evidence based assessments and interventions alongside your team of healthcare professionals to deliver a service for mental and physical health. You will provide consultation based services to teams and patients.

To be Successful in this role you will have:

  • The ability to support the general practice team with mental health and addictions
  • An understanding of behavioural health and deliver high-quality interventions
  • A focus on prevention and disease management
  • Professional accountability and an understanding of the standards and quality of care that needs to be adhered to, including legal guidelines
  • A desire to strive for continuous improvement with quality and risk management

You will need:

  • Current Annual Practicing Certificate
  • Clinical experience in a mental health setting
  • Experience practicing as a mental health professional in another setting
  • Registration as a current health professional (psychologist, nurse, occupational therapist, social worker
